Here are some helpful points to consider for your assessment.
After requesting an assessment through My Aged Care, an aged care assessor will visit you at your home to complete an in person assessment. Here’s how to register for an assessment if you haven't already.
You can ask a friend, family member, or carer to be there with you during your assessment. You can also have a translator or Auslan interpreter to support you.
What to prepare
Before your assessment you should have ready:
- Your Medicare card and other forms of proof.
- Copies of any referrals from your General Practitioner (GP).
- Contact details of your GP and other health professionals.
- Information on any other support you currently receive.
You can also prepare any questions you have about aged care to ask your assessor.
What happens during an assessment
At the start of your assessment, your assessor will ask for your consent and for permission to talk to the people close to you like your family.
They will ask you about your health, home and daily activities. This could include:
- Any medications you are taking.
- Any support you currently have.
- Social and cultural activities you like to do.
- How you do daily activities around the house.
- If you have problems with your memory.
What happens next
If you are eligible for Commonwealth Home Support Programme (CHSP) funding your assessor will let you know at the end of your assessment. For Support at Home, you will be notified if you are eligible after your assessment.
