Care management is an important service that ensures the quality and safety of your care.
It helps you get the most out of your aged care services, and it takes some of the stress out of planning and coordinating services, and support that you need.
With Silverchain, you will have a dedicated and experienced Care Partner to help you. This person is your key contact. They will make sure your care is safe, effective, and respectful of your preferences, culture, spiritual needs, and identity.
Your Care Partner will work with you and your family to create a Care Plan based on the Support Plan from your aged care assessment. We can also work with your health care providers to develop this. Your Care Plan includes:
- your goals
- how you would like your care delivered
- the services you will receive.
Your Care Partner will review your Care Plan regularly, and to make changes to your services to match your goals or needs over time. They can also help you access resources or other services like advocacy support to make informed decisions about your care.
Your Care Partner will check in with you monthly to see how you’re going, and you can contact them when you need.
If you’re receiving Restorative Care, you will have a Restorative Care Partner who is a qualified health practitioner. They will work with your other Care Partner and doctors to keep your services consistent.
Provider vs self-management
Under Support at Home, you can choose to receive care management services from your provider, or you can self-manage your care. Self-management means you plan, find and organise your own services, suppliers and manage your budget.
At Silverchain, all of our clients receive Care Management services. We manage your services for you, work with you to maximise your budget, and proactively review your care as your needs evolve.
Our compassionate Care Partners have extensive industry experience and connections to do this.
Care management funding
A Care Management Fee is set by the government at 10% of your quarterly budget if you receive ongoing services. This amount is the same for all providers, even if you self-manage your own care. See more information on fees here.
This fee is now lower than what was charged under Home Care Packages (HCP), where providers could charge different prices of up to 20% of your package level.
You may be able to apply for additional care management funds if you have specific or complex support needs. This can include being homeless, an Aboriginal or Torres Strait Islander, or a veteran with mental health conditions.

Refer to us
Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.
For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
