Receiving the right health care at home is not only good for your peace of mind but can also help you recover faster.

At Silverchain we provide a range of simple to complex health care services in the comfort of your home, at no cost.

Our services include nursing support to:

• Take the right medications, manage your incontinence, and heal a range of wounds.
• Manage chronic conditions like diabetes and chronic obstructive pulmonary disease (COPD).
• Connect with the care you need and manage your chronic disease conditions through care coordination services.
• Receive hospital level care and palliative care services.

How to access our services

For most of our services, you will need to be assessed by a health care provider to obtain a referral.

Depending on the service, you can receive a referral from a hospital clinician, your General Practitioner (GP), or specialist. Please check the eligibility information on the service you are interested in.

For some of our services you can self refer or receive care without a referral. If you have any questions, you can call our Contact Centre on 1300 650 803 or complete our enquiry form.
