As part of the Support at Home program, the Restorative Care Pathway provides higher levels of funding to help you stay well and out of hospital.
You can access up to $6,000 for Nursing Care, Allied Health services, and other Everyday Living Supports like Domestic Assistance for up to 16 weeks, if you are eligible. This funding is provided in addition to any ongoing Support at Home funding you already receive. If you need more care after 16 weeks, you can ask your Care Partner to review your Support Plan. If your amended Support Plan is approved, you may receive a further $6,000 in funding, with the maximum you can receive is $12,000 in total.
Our Restorative Care services
Our Allied Health team can support you to improve your mobility and wellbeing in person, over the phone or in a video call.
Our Nursing Care can include helping you with simple to more complex health care needs such as wound care or managing your medications.
You will also have a dedicated Restorative Care Partner to help you manage your services, set goals, and develop a tailored Care Plan. If you’re receiving ongoing Support at Home services, your Support at Home Care Partner, and Restorative Care Partner will work together to manage your care.
While you’re getting back on your feet, we can also arrange for Everyday Living Supports like Domestic Assistance, Home Maintenance, Meal Preparation and Transport.
Availability
Our Restorative Care services are available in all states in Australia including the Australian Capital Territory. You can also access these services for a fee for service. Availability varies by location, so contact us to see what is available in your area.
Eligibility
To receive Restorative Care Pathway funding you need to be eligible for Support at Home. You must also need additional, intensive Allied Health or Nursing Care for a limited time. If this applies to you, ask your care team to arrange an assessment. You will not be able to receive Restorative Care while receiving End of Life Pathway, Transition Care Program, residential aged care or residential respite services.
Costs
Restorative Care can include a mix of Clinical, Independence and Everyday living services.
The Australian Government expects all people to contribute towards the cost of their care, if they can afford to do so. All clinical services you receive under Support at Home do not require any contribution from you. This means you will have no out of pocket costs for these services.
