Get the support you need at home, whether you’re recovering from an illness or looking for ongoing care.
There are many benefits to receiving support at home as you age. These include:
Care and support when you need it helps you to age well
With some regular support you can continue to pursue your interests or hobbies, social life, and connections with family and friends. Continuing to do what you love is good for your wellbeing, keeping your mind and body active.
You are in control of the services you receive
With Silverchain you can start and stop services to take a holiday, and you can also have your family and carers involved in your care if you wish. It is all up to you.
Manage your health long term
Receiving our Best Care when you need it goes a long way. Regular help from a care worker, nurse or allied health care professional can help prevent a decline in your health or unexpected accidents, such as falls.
If you find it difficult getting dressed, eating well, or keeping your home safe and clean, these are important things to consider as they help you maintain your health and wellbeing.
Support and care that matches your needs
Home care services are designed to meet your needs and preferences. As part of the aged care assessment process, your needs are reviewed to determine the level of support that will help you maintain or improve your wellbeing, independence and quality of life.
There are many aged care providers and support options available to help you live independently at home. Start by thinking about what support would make daily life easier, whether that's help with household tasks, personal care, or managing your health.
It can also help to talk with your family, friends or GP about your needs, goals and preferences. They can provide support, help you understand your options, and work with you to plan for the future so you can continue living the way you want.
Plan ahead
There is often a wait to be assessed for Government aged care funding. If you need support now, or while waiting for your assessment, you can self-fund the services you need. With privately funded care, you do not need to meet any Government criteria or be assessed by My Aged Care.
