Our specialised dementia service is designed to help maintain and improve your memory, so you can manage everyday tasks and live independently.

One of our occupational therapists who specialises in dementia care will visit you at home to observe the tasks you can do. From there we will work with you and your carer to set long term goals.

We can help you to: 

  • Improve your ability to do regular tasks and maintain your existing skills.
  • Use strategies to improve your memory so you can be more independent.
  • Manage changes in your home that affect your ability to look after yourself.
  • Understand information about dementia so you and your carer can manage your condition in the future.

Our team can also provide training and support to carers or loved ones to help them better support you, and we can review your care plan whenever you may need it.

Refer to us

Health professionals can refer patients for government funded aged care by contacting My Aged Care online form, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122
