Our team can help you access other health services to meet your needs, like specialist care or community health programs. We work together with your doctors or other health practitioners to make sure that we provide you with the best care.

Support to manage health conditions

We help you improve your health and manage a range of chronic diseases and conditions. This includes diabetes, asthma, diabetes and incontinence. 

Medication management

Our team can help you take your medication correctly, and teach you how to monitor your symptoms, so you can manage your health better.

Wound care

Using clinically proven methods, we treat simple to complex wounds like skin tears, abscesses, pressure injuries, ulcers, and burns.

End of life care

We provide End of Life and Palliative Care in partnership with your medical practitioner to help you feel comfortable and supported during the final stage of your life. This can include nursing services like pain and medication management, as well as other services likes allied health or personal care.

We also provide nursing as part of various state funded health care programs.

Refer to us

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
