The Transition Care Program (TCP) provides short term funding for services designed to help you get better after a hospital stay.
To be eligible for TCP you will need to be aged 65 and over, or aged 50 and over for Aboriginal and Torres Strait Islander people. You must also be a current hospital patient ready for discharge and have a need for short term support. Services can be provided for up to 12 weeks in your home or in the community.
At Silverchain, we provide TCP services in the Great Southern in Western Australia (WA), and in the Sunshine Coast region in Queensland. They include:
- Personal Care
Support with daily self care tasks like showering, getting dressed, and taking your medications. - Allied Health and other therapeutic services
Support to restore and maintain your strength, mobility and balance, so you can live safely and independently. - Nursing Care
Personalised care to meet a range of simple to complex health needs.

To access TCP support, you need to be assessed while you’re in hospital. You can ask your hospital staff to organise this, or you can schedule an assessment through My Aged Care.

Refer to us
Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.
For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
