When you change to Silverchain, you should not need another assessment, and we will help you transition your care arrangements so there is minimal delays. We will help you transition your care with minimal delays.
Here’s how to receive care from Silverchain:
1.Contact your current provider and cancel your services
To change providers, you will need to let your current provider know that you want to change, and you will need to agree on an end date for your services. They will advise Services Australia. Only cancel your current services with them, do not cancel your funding package. Otherwise, there may be a gap in your care.
2. Contact us and sign your Service Agreement
Contact us and let us know that you would like Silverchain to be your provider, and we will organise a care team member to visit to you at home. We will explain our Service Agreement, pricing and costs, and answer any questions you have about our services.
If you’re changing providers, we can agree on a start date that is the same as your current services, or afterwards.
3. Create a Care Plan and budget
Your new Care Partner will work with you to design a Care Plan based on your assessment by My Aged Care and your budget. If you need to do an income and asset test, we can talk you through how to do this.
4. Continue receiving services until the end date
If you’re changing providers, you should continue to receive their services until the end date you agreed. They will then give you a final invoice and statement before your services finish. They will also share information about your services with us. This includes any assistive technology or home modifications you’re waiting on.
5. Start receiving our care
Your Silverchain services will start on an agreed date. Your Care Partner will work with you to make sure your needs are met and that there is no a gap if you’re transitioning your care to us. If you have a transitioned from a Home Care Package and have unspent funds, your provider has 70 days from your end date to transfer these funds to us.
