This new program aims to improve your access to aged care services, products and equipment to stay healthy and connected at home, and in your community.

Support at Home brings together existing government subsidised in home aged care programs into a single program. It replaces the Home Care Package (HCP) and Short Term Restorative Care (STRC) programs. 

There are eight levels of ongoing funding to meet your needs, instead of four like HCP. As your needs change, you can be re-assessed to a higher level and receive more funding. There are also short term pathways to meet different needs:

You will have a Care Partner to help you organise your care and services to meet your needs. 

With your Support at Home funding you can access range of our services from a little help at home to complex health care. These include:

Clinical supports
Independence supports
Everyday living supports

Contributing to your care

Most people on Support at Home will be asked to financially contribute towards the cost of their care. Learn more about your contributions.

Transitioning to Support at Home

From 1 July 2025, people currently receiving or assessed as eligible for HCP or STRC funding will transition to Support at Home. Learn more about these changes and how they may impact you.

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
