This new program aims to improve your access to aged care services, products and equipment to stay healthy and connected at home, and in your community.
Support at Home brings together existing government subsidised in home aged care programs into a single program. It replaces the Home Care Package (HCP) and Short Term Restorative Care (STRC) programs.
There are eight levels of ongoing funding to meet your needs, instead of four like HCP. As your needs change, you can be re-assessed to a higher level and receive more funding. There are also short term pathways to meet different needs:
- Restorative Care pathway: Short term support to maintain and improve your independence through multidisciplinary allied health services.
- End of Life pathway: Additional funding to access services and remain at home during your last three months of life.
- Assistive Technology and Home Modifications (AT-HM) Scheme: Access to products, equipment and home modifications to help you live independently at home.
You will have a Care Partner to help you organise your care and services to meet your needs.
