The Assistance Technology and Home Modifications (AT-HM) Scheme supports you to purchase or loan products, equipment and home modifications to help you stay at home.
Funding is available through Support at Home and includes funding to buy equipment like wheelchairs and devices to help you get out of bed and to move around. It also covers modifications to your home like handrails or ramps.
There are three levels of funding (called tiers) you may be eligible for:
- Low tier: $500
- Mid tier: $2,000
- High tier: $15,000.
You have up to 12 months to use your funding, which is in addition to your Support at Home funding.
The Australian Government has a list of products, equipment and home modifications available for purchase or loan for you to choose from.
If you have specific needs like a permanent disability, you may be eligible for additional funding for a longer period. Your allied health team or Care Partner can provide you with advice on this.
Silverchain’s team of allied health professionals support our clients by recommending suitable home modifications and equipment to live safely at home. We also help set up your modifications and equipment and provide advice and support on how to best use them to improve your wellbeing.
We provide equipment and home modifications in all states and the Australian Capital Territory through the Support at Home AT-HM Scheme.
Eligibility
You will need to do an aged care assessment through My Aged Care to determine if you are eligible for the AT-HM Scheme.
If you are transitioning to Support at Home from a Home Care Package (HCP), you can talk to us about your needs. There are some special arrangements in place that we can help you with.
Contributing to your care
You will need to financially contribute towards the costs of your equipment or home modifications. Services Australia will determine how much you need to pay.
Allied health services are clinical services, and under Support at Home, any clinical service you receive are fully funded by the Australian Government. See more information on contributions.

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.
For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
