Our allied health team aim to keep you active and mobile while ensuring your home is free of hazards that may put you at risk of falls. We take a holistic approach by assessing your current health and condition, as well as your home for risks.

  •  Home hazard safety checks.
  • Health and wellbeing checks which look at your overall nutrition, medical condition and mobility. 
  • Advice and support from dietitians to ensure you are eating well.
  • Support from Physiotherapists to develop an exercise plan to rebuild or maintain your strength and balance and help prevent falls.
  • Occupational Therapist support to access prescribed assistive technology, equipment and home modifications. 
Assistive technology and home modifications

Any equipment or product to help you do your everyday activities is called Assistive Technology. This can include:

  • Mobility equipment like walking sticks and wheelchairs.
  • Toileting devices such as bedpans.
  • Items to help you eat like modified cutlery.

Home modifications are changes to your home to make it safer and more accessible. This can include simple modifications like grab rails and or modified door locks, or larger modifications like ramps. 

If you need more complex assistive technology or home modifications, our experienced allied health professionals can help you access what you need. We can also:

  • Organise the setup of your equipment or modifications.
  • Teach you how to you use them properly.
  • Organise any repair services if needed.

Refer to us

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
