Silverchain has been providing care for people living with life limiting illnesses for over 40 years. 

We specialise in providing compassionate and sensitive care to meet your physical, emotional, spiritual and social needs. Our End of Life Care is provided in partnership with your doctor and our team, to help you feel comfortable and supported.

It includes Nursing Care such as pain and medication management, as well as other services like allied health care for help with your mobility, or Personal Care for help with things like showering, dressing and personal hygiene.

You can receive End of Life Care for days, months or for up to a year, depending upon your needs. At Silverchain, we also provide Specialist Palliative Care at home, depending upon where you live.

The difference between Specialist Palliative Care and End of Life Care

End of Life Care is the last stage of Specialist Palliative Care. Its focuses on helping you feel supported and pain free towards the end of your life. 

Specialist Palliative Care is suitable if you receive a life limiting diagnosis. The length of time you can receive care varies for everyone, but is generally longer than End of Life Care and can be for several years. You may also come in and out of Palliative Care. 

Specialist Palliative Care helps you manage your symptoms, so you can enjoy a good quality of life.

Advanced Care Directive, or an End of Life Plan

An Advanced Care Directive (also called a Living Will) is a Care Plan that includes your health and personal preferences if you can no longer communicate this yourself. This is an important part of End of Life Care, and is designed to help your health professionals and family make decisions for you if you can’t. Your Advanced Care Directive appoints someone as a substitute decision maker to make choices for you when the time comes. 

As part of our services, we support clients and their families to think about advanced care planning. We can also help you develop an Advanced Care Directive.

Refer to us

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
