At Silverchain, we provide three types of care: 

Clinical support

Specialised services to help you maintain your abilities and improve your health to live well at home. Our services are delivered and/or supervised by qualified health professionals. 

  • Nursing Care: Personalised care to meet a range of simple to complex health needs like wound care and incontinence.
  • Allied Health and other therapeutic services: Support to restore and maintain your strength, mobility and balance, so you can live safely and independently.
  • Nutrition: We can review your diet and prescribe specialised products and equipment so you can get the nutrients you need.
  • Care Management: A dedicated Care Partner to help plan, coordinate and review your services to meet your goals.

Refer to us

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
