We provide a range of services through CHSP to support you at home.

You may be eligible for CHSP funding if you need some support to improve your health and wellbeing. You will also need to be aged 65 and over, or 50 years and over if you’re an Aboriginal or Torres Strait Islander.  

The services we provide include a little help with daily tasks like preparing meals, through to nursing care. They are grouped into three types of care:

Clinical Support
Independance support
Everyday living support

Our services are available in all states across Australia and in the Australian Capital Territory. 

Service availability varies by location, so contact us to see what is available in your area. If you do not have CHSP funding here’s how you can apply for an assessment. For more details, visit the My Aged Care website. 

If you need more care than what CHSP can provide, you may be eligible for Support at Home. You will have more flexibility and access to a wider range of complex services. You will also have a dedicated Care Partner to help you organise your care and services to meet your needs.

Refer to us

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
