The funding from the Australian Government in your Support at Home package may include:

Your funding will be split evenly across the year for you to pay for services you need. You will receive a quarterly budget every three months to pay for the services you need in line with your Support Plan.

If you have assistive technology or home modifications through the AT-HM Scheme, you can spend the full amount of funds straight away.

Existing HCP clients can keep any unspent funds for use under Support at Home. This money can also go towards assistive technology, home modifications, or short term supports if needed. 

Rolling over funds

Unspent funds of up to $1,000 or 10% of your budget (whichever is higher) can be rolled over to the next quarter for ongoing services. This means you can pay for additional care if anything unexpected happens, like if your carer is unable to support you, or you are discharged from hospital.

You cannot rollover any funds for short term pathways such as Restorative Care, or any unspent funds greater than the $1000 or 10% of your total package.

Your financial contribution

Under Support at Home, you will be asked to financially contribute to the cost of your care if you can afford to. Learn more about your contributions.

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
