Restorative Care can help you get back on track if you had a hospital visit, illness, or injury.

This pathway replaces the Short Term Restorative Care (STRC) program. It provides higher levels of funding and is designed to help you avoid going to hospital.  

You will have $6,000 (or up to $12,000 if needed) for support like allied health services for up to 16 weeks. You can also access equipment and modifications through the Assistive Technology and Home Modifications Scheme.

Restorative Care is available in addition to any Support at Home funding you receive. If you need more support after 12 weeks, you can ask your Care Partner for a Support Plan review.

At Silverchain, our allied health team are here to support you to improve your mobility and wellbeing. Our team includes experienced physiotherapists, occupational therapists, podiatrists, social workers, dietitians and speech pathologists. We can provide care in person, over the phone or in a video call.

You will have a dedicated Restorative Care Partner to help you manage your services, set goals, and develop a tailored Care Plan. Your Care Plan can include steps to work towards your goals and deal with common issues. 

If you’re receiving ongoing Support at Home services, your Support at Home and Restorative Care Partners will work together to manage your care.

Availability and eligibility

We provide services across all states in Australia and the Australian Capital Territory, through the Restorative Care Pathway. You can also access these services privately. Availability varies by location, so contact us to see what is available in your area.

To receive Restorative Care you need to be eligible for Support at Home. You must also need additional, intensive allied health or nursing services over a short period. 

You will not be able to receive Restorative Care while receiving End of Life, Transition Care Program, residential aged care or residential respite services.

Contributing to your care

Restorative Care can include all types of aged care services. Under Support at Home, any clinical services you receive are fully funded by the Australian Government. See more information on contributions.

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.

For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
