If your health has changed recently or you have returned from hospital, you might find it harder to do your regular tasks. With Short Term Restorative Care (STRC) funding, you can receive care to help you regain or improve your independence. This allows you to live confidently and comfortably at home.
STRC services are designed to help you achieve your set goals. They can be provided in your home or in the community over eight weeks.

We provide the following STRC services to clients in the South West and Great Southern regions of Western Australia (WA):

Health professionals can refer patients for government funded aged care by contacting My Aged Care, or by calling 1800 200 422. You can also make an e-referral from your practice management system.
For private services, patients do not require a referral or assessment and can contact us directly. However, you can contact us on a patient’s behalf by calling 1300 300 122.
