Join Silverchain’s panel of aged care experts to understand the Australian Government’s Support at Home Restorative Care Pathway, and how it can support older people to regain or maintain their physical function and independence.

The session will provide information on the pathway, including eligibility criteria, funding, costs, how to identify patients who may benefit, and the referral process.

Using practical examples, our team will demonstrate how assessment, treatment, and support can be tailored to a patient’s home environment, treatment goals, and capacity.

Access our exclusive pre-recorded webinar on Thursday 12 November 2026 at 6pm to 7pm AWST/ 8pm to 9pm AEST or stream the recording afterwards on demand.

The session is eligible for one educational activity CPD hour.

Our team will consist of Silverchain Head of Policy and Advocacy, Dr Ronelle Hutchinson, supported by our General Manager of Care Partnering, Cassandra Tobin, Head of Multidisciplinary Care, Christine Chipchase, and GP Liaison Dr Erin O’Donnell-Taylor.
